You have navigated to the Streets Department site setup for application documentation submissions. Application submissions for all Pole Attachment requests must be packaged into one singular PDF file per pole request and include the following items:
- • Proper Application Form (for either utility pole or city pole attachment)
- • Map showing the proposed location of the Facilities with identification of any park, school or Historic building within 300 feet of the proposed location.
- • Actual Pole Site Photos, and representative drawings of the Communication Antenna Facilities as they will look when installed, including the immediate surrounding area.
- • Proposal for collocation, if collocation is requested.
- • Documentation of permission between Provider and utility owner that Provider has authority to attach to Utility Pole.
- • Identification of proposed location of connection to electrical supply or of any fiber connection required, including the identification of any planned interconnection with the Facilities of any other Providers.
- • RF Emissions Report and Noise Report confirming that all Communication Antenna Facilities, and associated equipment, meet all applicable legal requirements.
- • Engineering design and construction plans.
- • Structural calculations, signed by a Professional Engineer, licensed in the Commonwealth of Pennsylvania, showing that the structure, including pole and foundation can safely tolerate the weight loads of proposed Communication Antenna Facilities.
